Please review this form to ensure all information is correct. All cancellations and changes must take place by phone 2 full business days prior to the event. The payment method must also be secured 2 full business days in advance. We accept University funds, cash, checks, Mastercard, and Visa. All events not paid through University funds are subject to sales tax.

A signed copy of this sheet with changes needs to be in our office 2 full business days before the event. Due to food safety and liability associated with proper food handling, it is our policy that no food or beverage be removed from the site of an event.
